FDNY report lists many problems at fatal WTC fire

The Fire Department of New York has issued a report that details a litany of problems at a toxic ground zero skyscraper that caught fire one year ago and killed two firefighters.

The report says there were inspection failures, communication lapses and many hazardous conditions that hampered the efforts to fight the fire at the former Deutsche Bank tower. The hazardous conditions included blocked stairwells and a broken standpipe that kept water from firefighters.

The report includes transcripts of every radio transmission, including 30 maydays.

It was released Thursday at fire department headquarters in Brooklyn; many of its details came out a day earlier in news reports.
